    Please enter the Make & Model of the club getting extended along with the requested specifications. You can also include any special notes about the build here.

    *A $30.00 repair services shipping fee is applied to all online repair services.  This fee includes a prepaid shipping label emailed to you, along with shipping back after services are completed.  The $30.00 fee will only be applied once to each repair services order. Be sure to combine multiple clubs and/or services to receive the very best deal on shipping.
